What changed since the Pixel Watch 5 launched?
Blood pressure and insulin resistance tracking were headline features when Google unveiled the Pixel Watch 5 in August, as covered in our Made by Google 2026 roundup. They were missing from the watch's first software update, which we noted in our report on the Pixel Watch 5 September update. Health Guardian is where they finally land, and Google has also widened the list beyond the new watch: Pixel Watch 3 and Pixel Watch 4 owners get the same three trend features at no cost.
The catch is timing. None of these features gives an instant reading. Each one needs weeks of wear before it reports anything, which is why the first summaries will not appear until early October.
How much wear does each Health Guardian feature need?
Google publishes a different minimum for each feature. Here is how the requirements and reports compare, taken from Google's announcements and the Google Health 5.09 release notes.

Blood pressure trends work without a cuff or manual calibration. The Pixel Watch passively measures pulse and motion patterns across the month, and Google says the aim is to spot persistently higher cardiovascular trends early. If the watch flags a pattern, Google's guidance is to confirm it with a home blood pressure monitor or a healthcare professional. Insulin resistance trends need no blood draw; Google says they run on its largest sensor foundation model, pre-trained on more than 1 trillion minutes of data from 5 million opted-in users.
Is Pixel Watch blood pressure tracking a medical test?
No. Google describes all three trend features as general wellness tools that are not meant to diagnose, treat or prevent any condition. Treat a flagged trend as a reason to check with a doctor, not as a result on its own. Google also states that Breathing Emergency Detection, the European-only feature, has not been cleared or evaluated by the FDA.
Google lists no country restriction for the three trend features. The only feature it limits by region is Breathing Emergency Detection. In Japan and Mexico, the blood pressure and insulin resistance features ship under different names: Vascular Stress Trends and Metabolic Stress Trend.
What else is in Google Health 5.09?
The Google Health 5.09 update is not only about Health Guardian. According to the release notes, Cardio Load can now count workouts recorded on other devices, such as a Garmin watch or an Apple Watch, as long as heart rate was captured during the session. The Fitness tab gets more options for logging a completed activity, missing step streak history is being restored, and creating custom foods now makes it easier to switch serving units and meal times.
